Patient Diary

The RCS Patient and Lay Group (PLG) has developed a patient diary for people who are having an operation.

The diary will help you record your thoughts, get ideas about the questions you may want to ask the surgical team, and monitor your progress and recovery. The diary can be used in conjunction with the Recovering from Surgery online resource and our ‘It’s OK to ask’ questionnaire.

The PLG has also developed It’s OK to ask, a questionnaire designed to help us find out about your experience of your operation. We believe that your experiences of your operation are important. As a patient you know when you have received really good care. However, you can also see where your care could be improved.

You can help by answering the questions in this document. Your answers will be added to those of others to see if there are areas that most patients tend to find go well, or find difficult. We can then use this information to help inform our work with surgeons to improve patient care.

Your responses will be treated anonymously.
